A HINTS bedside case

A 65-year-old man presents with continuous spinning vertigo for 6 hours and persistent horizontal nystagmus that beats to the right. What do the three HINTS findings suggest?

Head impulse

Catch-up saccade when head turned to one side?

Nystagmus

Direction of the fast phase across gaze positions?

Test of skew

Vertical refixation on alternate cover?

Voice Alterations Associated with Implanted Vagus Nerve Stimulation in Epilepsy: A Systematic Review and Meta-Analysis

Vagus nerve stimulation for drug-resistant epilepsy consistently reduced maximum phonation time by ~9 seconds and significantly worsened Voice Handicap Index scores compared to controls, alongside increased jitter, shimmer, and perceptual voice deterioration. Routine voice monitoring should be incorporated into long-term management of VNS patients.

Role of Laryngopharyngeal Reflux Disease in Eustachian Tube Dysfunction: A Systematic Review

Laryngopharyngeal reflux was present in up to 58% of adults with Eustachian tube dysfunction and 77% with otitis media with effusion, with pepsin detected in 40% of middle ear effusions, suggesting a probable association; however, methodological heterogeneity across 20 studies prevents causal conclusions.

From signals to phenotype: multifactorial drivers of tumor-associated macrophage polarization in oral cavity squamous cell carcinoma: a narrative review

In OSCC, tumor-associated macrophages form functionally diverse, spatially organized populations driven by hypoxia, lactate, cytokines, cancer-associated fibroblasts, and microbial signals rather than a simple M1/M2 dichotomy. CD163- and CD206-high macrophage infiltration correlates with adverse clinicopathologic features, and preclinical TAM-targeting strategies reduced tumor-promoting activity but require prospective validation before clinical use.

Molecular targeted therapy for thyroid cancer in Japan: Current status and future perspectives

Molecularly targeted therapies for RAI-refractory differentiated thyroid cancer in Japan have shifted from histology-based MKI selection toward genotype-directed treatment using BRAF/MEK, RET, and TRK inhibitors, though mutual exclusivity of driver alterations limits each agent's eligible population. Optimising sequencing of MKIs and targeted agents alongside comprehensive genomic profiling is now central to improving outcomes.
